Abstract: At approximately 11:53 am, on September 29, 2010 Employee #1, a machine operator of Real Mex Foods suffered an amputation to his right middle and ring finger due to cleaning a food pump while the pump was still running. Employee #1 was cleaning a food pump in the attempt to clean some dough from the food pump. He was cleaning a Waukesha Cherry-Burrell Pump, Serial #370320 04. At the time of the accident the food pump was energized and not locked out. Employee #1 was not using an extension tool to remove food from the pump. Employee #1's fingers contacted the rotors of the food pump causing the amputation.
