Abstract: At approximately 10:35 a.m. on February 27, 2008, Employee #1, a laborer, was operating a long conveyor, installing tips and caps on aerosol cans. The conveyor would not start properly by turning a ball valve to supply compressed air to the air motor that moved the conveyor. This was a common occurrence, and the employee followed the procedures she had been taught to fix it. She disconnected and reconnected the compressed air supply hose to the compressed air supply to the ball valve and manually pulled the conveyor drive chain between the air motor and the conveyor belt. As the employee pulled the drive chain, the drive motor quickly started, pulling her fingers in between the drive chain and sprocket at the air motor. The four fingers on her right hand were amputated at the knuckles. Employee #1 was hospitalized and underwent surgery at a local medical center. She was not working in close proximity with any coworkers, so there were no witnesses. Interviews were conducted of the injured employee and her supervisor. The investigation concluded that the drive chain and sprocket were not effectively guarded as required. As a result, the Division issued a serious accident related citation for a violation of T8 CCR 4075(a).
