Abstract: On November, 09, 2007, at approximately 8:30 a.m., Employee #1, a general labor employee of California Plant Services, gets injured when he climbs into the topcap of an 8 feet A-frame ladder to access a crawl space that was approximately 12 feet above ground. Employee #1 was making a galvanized steel electrical box frame. The task requires the use of special nuts. Employee # 1 asked Employee # 2, his supervisor, for the special nuts and was told that they were located in a crawl space that was approximately 12 feet above ground. Employee #1 goes to the area where the crawl space was located and sees an 8 feet ladder that belongs to Employee # 3, a contractor that was on site. Employee # 1 climbs the 8 feet ladder and stands on the topcap of the ladder and pulls his body up the rest of the way to access the crawl space. When Employee # 1 comes down from the crawl space he steps onto the top cap of the ladder and as he places his other foot the ladder tips over causing Employee # 1 to fall approximately 12 feet to the ground. Employee # 1 sustains an injury of multiple fractures to his left elbow. Employee #1 was transported to White Memorial Hospital by Los Angeles City Fire Department where he was treated for his injuries. Surgery was performed to repair the fracture and was hospitalized for five days.
