Abstract: At approximately 11:30 a.m. on April 30, 2007, Employee #1, a construction laborer, was erecting concrete shoring forms. Part of this process involves placing wood blocks in the shoring system for height and stability of the forms. Employee #1 was assigned to place wood blocks cut from 2-in. by 4-in. and 4-in. by 4-in. lumber, in the concrete shoring system of building C. He was working with a coworker that was approximately 30 to 40 ft away. Employee #1 had already cut approximately 20 to 30 of the blocks with a Skill saw. He needed a shim and was attempting to make the wood shim by splitting a piece of 0.625-in. thick plywood that was 3-in. in width and approximately 57-in. long. He placed the plywood on a make shift worktable and was holding the plywood with his left hand, so the thickness part of the plywood was resting on the table. Holding the Skill saw with his right hand he then proceeded to lower the Skill saw's rotating blade into the plywood, but the Skill saw kicked backed, cutting his left thumb. Employee #1 was hospitalized.
