Abstract: Around 10:30 a.m. on August 24, 2007, Employee #1, a maintenance mechanic, was adjusting a motor brake. He was evaluating the cause of a machine malfunction, causing the machine to lower wooden pallets from the storage position onto the machine, without a command from the operator. Employee #1 unbolted and removed the outer steel panels of the machine to gain access to the internal mechanisms. He adjusted the brake on the pallet lifting motor and was looking for the other possible problems within the machine. In order to access the interior of the machine, Employee #1 had to lean over a horizontal gear rack that ran along the machine interior. While Employee #1 was leaning over the gear rack, with his hand on the rack, the maintenance manager instructed the operator to try the machine. When the operator started the machine, the pinion gear traveled forward, over the right hand of Employee #1, injuring his fingers on his right hand. The tips of his second, third, and fourth fingers were amputated. Employee #1 was transported to Western Hand Center. After a brief examination at the Technimed Industrial Clinic, Employee #1 was treated and released. Based on the investigation, it was concluded that the machine was not locked off by Employee #1 before he adjusted the brakes. Employee #1 denied being trained on the requirements of lockout/tagout.
