Abstract: At approximately 4:00 p.m., on October 10, 2005, Employee #1 was performing preventative maintenance on a baking pan line at the Freund Baking Company. The preventative maintenance included inspection and manual cleaning of the gear and sprocket and drive shafts of the conveyor motors which are located approximately 8 ft from the floor. In order to access the motors, Employee #1 used a 4 ft step ladder. While standing on the second to the top step of the ladder, he began cleaning the shaft of the motor with a rag. The line was still energized and running. To access the drive shaft, he had to reach over the gear and sprocket drive, which was not equipped with a guard. The rag became caught in the chain drive. When he tightened his grip on the rag to try to retrieve it, his hand was pulled into the chain and sprocket drive, causing an injury that required amputation of his finger. The employer did have energy control procedures in place for locking and tagging out the equipment. Employee #1 had been trained in the procedures; however, he did not implement effective energy control procedures for the safe cleaning of the pan line drive shafts.
