Abstract: At approximately 12:20 p.m. on September 6, 2005, Employee #1, an iron worker, was measuring rebar with a tape measure while standing on the wooden floor covering on the deck. The dimension of opening was 94 in. by 125 in. and it was covered by two wooden plates. The wooden covers were located 1 ft below the deck level. Employee #1 moved away from the area he had been working to drink water. As Employee #1 stepped back on the wooden floor covering after drinking water, one of the wooden plates (94 in. by 50 in.) gave way and Employee #1 fell approximately 13 ft through the opening to the concrete floor. Employee #1 sustained a lumbar compression fracture and left radial fracture of the ulna. Employee #1 was hospitalized for 15 days.
