Abstract: At approximately 9:30 a.m. on September 16, 2005, Employee #1 was clearing land with a front-end loader when he stopped and climbed down to smoke a cigarette. Upon finishing the cigarette, he tossed it into a pile of debris which, unknown to the employee, contained an accelerant. The debris pile ignited at a very rapid rate and Employee #1 received burns to 30 percent of his body. Employee #1 was hospitalized and died 36 days later from his injuries.
