Abstract: At approximately 11:00 a.m. on July 17, 2008, Employee #1 was pushing a pallet jack that had approximately 10 doors on the pallet jack. It was approximately 93-degrees Fahrenheit. Employee #1 stated that he felt dizzy and lightheaded. Employee #1 went to his truck and turned on his air conditioner. Employee #1 was still feeling a little dizzy and lightheaded. Employee #1 was transported to the hospital where he diagnosed with heat exhaustion and dehydration. Employee #1 was treated and released on the same day of his treatment.
