Abstract: At approximately 2:50 p.m. on August 17, 2007, Employee #1 was working for a steel fabricator. A coworker was operating a Gradall forklift to stack 1900-pound steel beams. The coworker was moving the last beam in this stack when it became hung up. The employee climbed up to the top of the banded stack of beams near the fence to "walk the beam," to find out why it was hung up. The beam became free, swung and hit both his legs at of the employee the same time. He fell off the stack of beams (approximately 7.5 ft), hit the fence and landed on some beams on the ground. Employee #1 was taken Arrowhead Regional Medical Center, where he was hospitalized for approximately five days.
