Abstract: At approximately 2:30 p.m. on June 14, 2007, Employee # 1 was cutting a track chain at a heavy equipment rental company. The rolled-up track chain measured 25 feet long, 10 inches wide, and weighing approximately 3,500 pounds. The chain was leaning against the tines of a forklift approximately three feet above the ground level. When Employee #1 made cuts on the chain using a welding torch, pieces fell to the ground. When Employee #1 walked in front of the remaining chain, it fell and hit him on his right thigh, causing him to lose his balance and fall on the ground. The track chain continued to fall, crushing Employee #1's left foot and lower leg, which was amputated.
