Abstract: At approximately 3:36 a.m. on February 24, 2007, Employee #1 was operating an order picker (Raymond Model Number 261-OPC3077, Serial Number 261-05-02987) to stock merchandize on the racks. Just before the accident, Employee #1 was at the second level of the rack (8-ft 2-in. above the ground level). Employee #1 had a bundle of plastic containers on the platform of the order picker. Employee #1 picked up the containers and put on the second tier of the rack. As Employee #1 pushed the containers into the rack, he stepped on a brown paper which was overhanging on the edge of the platform. Employee #1 lost his balance and fell on to a concrete floor. Employee #1 was not using fall protection at the time of accident. Employee #1 sustained brain injury and a fracture on his left scapula which required hospitalization for 32 hours.
