Abstract: At approximately 8:00 a.m. on January 24, 2007, Employee #1, a stone slab installer, was unloading and stacking granite slabs at the storage yard of the manufacturing facility. Employee #1 was holding one side of a granite slab (109-in. long by 65-in. high by 0.75-in. thick; and weighing approximately 500 pounds to 650 pounds). The slabs were being unloaded from a parked flat bed truck and stacked on an A-type storage frame. The slab was held by an "Abaco Lifter" clamp attached to the 8-ft boom of a "Yale LP-type" fork lift operated by a shop foreman. The operator picked up the slab and moved it off the trucks' flat bed by driving the fork lift in reverse. At this time, the operator began driving the fork lift in the forward direction, the slab swung. Employee #1 tried to stop the slab from moving away when the forklift's front right tire ran over both his right and left ankles and feet. Employee #1 sustained serious compound fracture injures to both right and left ankles. Employee #1 underwent surgery and was hospitalized for five days.
