Abstract: At approximately 9:30 a.m. on January 8, 2007, an employee, a carpenter, along with coworker was assisting a forklift operator in loading trusses onto a one story restaurant under construction. The forklift operator was operating a variable reach rough terrain forklift truck, Sky Trak, Model Number 10543, Serial Number 13833, equipped with a 12-ft truss boom, Sky Trak, Model Number 7119700, Serial Number 700106, while the coworker was standing at a doorway relaying signals between operator and the employee, who was inside the structure. They loaded one bundle of trusses onto the structure without any incident. As the second bundle was being lowered into the structure, the truss support plate that ran mid-span between the exterior walls collapsed and the first bundle came crashing down. The employee was struck by the bundle and was injured seriously. The bundle that struck the employee was made of open web metal plate connected wood trusses. The temporary support failed under the excessive loading of the 51-ft long and horizontally loaded truss bundles. The employee was hospitalized for unspecified injuries.
