Abstract: On June 14, 2006, Employee #1 was filling a storage container with sand from his pneumatic trailer at a concrete vendor's facility. The employee had been using the internal air blower system on his truck to transfer the sand from the second pneumatic trailer into a 4,041 cubic foot mobile storage container, called a "guppy." The product transfer had stopped before completion, so Employee #1 shut down the trailer's pneumatic system to investigate. However, he did not check the pressure gauges on the "guppy" or relieve the residual back pressure present before he disconnected the fill hose from the trailer's product discharge line. Residual air pressure vented out through the hose, causing it to strike his lower right leg. The employee's leg was fractured and he was hospitalized.
