Abstract: At approximately 12:30 p.m. on July 13, 2006, Employee #1, a union electrician, was working on an electric pole with a coworker, transferring the service lines (120/240-Volts, secondary lines) from old poles to new poles. Employee #1 and his coworker were working in a man basket (Aerial Device). While transferring the last service line, Employee #1 shorted the two secondary terminals (phase to neutral) with his left arm, which caused a flash. He received flash burns on his left forearm, which required skin grafting and hospitalization for over 24 hours.
