Abstract: At approximately 9:00 a.m. on May 4, 2005, Employee #1 was using a Delta Rockwell 12 in. to 14 in. tilting arbor saw, Model Number 34-395, Serial Number 1576242, to cut a 0.5 in. piece of plywood for a customer. The saw blade was raised 2.5 in. The saw had a hood guard that could enclose the saw blade. Employee #1 was wearing gloves, was not using the cutting guide and was cutting the plywood without the guard in position. While cutting, the wood lifted above the blade. Employee #1 was injured when he raised his left hand to control the piece of wood and his fingers were caught by the moving blade. He was treated at the hospital for the amputation of his left pinkie and ring finger.
