Abstract: At approximately 9:00 a.m. on August 23, 2006, Employee #1 was working for a trucking, picking up a load of lightweight aggregate from a surface mine. While he was placing a tarp over the load in the rear trailer of the Mack transfer dump truck he was operating, he fell about 9 feet to the ground. Employee #1 was hospitalized and treated for multiple fractures to his right leg. The accident investigation revealed that the mine site, where Employee #1 was working, requires drivers to use fall protection (harness, lanyard and horizontal life lines) that must be attached to a permanent platform. This requirement applies to all workers that are engaged in cleaning, tarping or leveling of loads. The cause of this accident was determined to be due to Employee #1's failure to use the provided fall protection.
