Abstract: On February 18, 2005, Employee #1, a carpenter, was working from a material box, measuring 8 feet by 4 feet by 4 feet, removing tools and equipment to the second floor. Employee #1 stated that he moved to the right of the box, causing it to capsize and fall to the ground. The employee suffered two broken legs, two broken ankles and a shattered heel. He was hospitalized for treatment.
