Abstract: At approximately 6:45 p.m. on May 13, 2004, Employee #1, a bakery machine operator, was operating a 410 AMF Pan-O-Mat machine that sizes and shapes bread dough to be deposited in a pan. He was working with a coworker who was feeding baking pans into the machine. Employee #1 was working at a mid-way point along the machines approximately 40 ft length when bread dough clogged the sheeting rollers that feeds the reshaped bread dough into the baking pans. Employee #1 was clearing the residue dough from the unguarded sheeting rollers by reaching into the machine and extracted the bread dough while the machine was running, a maneuver he has completed many times without incident. His right hand was caught in the machines unguarded sheeting rollers approximately 5 inches in diameter, 30 inches long, and 48 inches above the floor. The coworker shutdown the machine by pushing an emergency stop at the entry but it only shutdown the belts and drive chain on that end of the machine. Employee #1 sustained extensive injuries to both sides of the index and middle fingers removing chunks of tissue that could not be repaired. The thumb was also lacerated and fractured, and he was hospitalized.
