Abstract: Employee #1 was using a 28-foot extension ladder to access a steel building. The ladder was extended to a length of 15 feet and placed against a steel beam. The employee started up the ladder to reach some bolts and when he reached the fifth rung, the feet of the ladder slid and he began to fall. His foot became caught on a horizontal beam, flipping the ladder over and causing him to fall head first 5 feet to the ground. The employee was killed in the fall.
