Abstract: On November 1, 2001, Employee #1, a superintendent at a residential construction site, and a coworker, were on a 10.5-ft-tall, job-built wooden work platform, pouring and vibrating concrete into a form at the west wall of the structure. The coworker was holding the vibrating machine and Employee #1 was vibrating the concrete when, without warning, Employee #1's knees buckled. He dropped to the floor of the platform and then fell backwards off the platform and onto the concrete floor. His head struck the floor as he landed and he suffered a fractured skull. Employee #1 was killed. Subsequent investigation determined that the concrete vibrator was operating properly, ruling out electric shock as a cause for the fall. Apparently, Employee #1, who had a history of cardiac problems, was not feeling very well that day and was coughing more than usual. The work platform was not equipped with standard railings and neither Employee #1 nor the coworker was using any form of fall protection.
