Abstract: On December 9, 2003, Employee #1 and Employee #2 were removing the outer wrapping from a 221 psi hoop-wrapped expired SCBA cylinder, using a Skilsaw with a carbide tip blade. They did not check the pressure inside the cylinder first. They placed the cylinder on the ground. They supported the cylinder on one side with a brick held in place by Employee #1's foot, and they braced the other side against a cast iron sewer pipe. Employee #2 began to cut the outer wrapping, starting from the base and going toward the cylinder neck, when he heard hissing and then a boom. The saw blade had punctured the aluminum cylinder, causing it to explode. The saw was shattered and was launched from Employee #2's hand. Employee #1 sustained a fracture and was not hospitalized. Employee #2 received a foreign body in his eye and was hospitalized.
