Abstract: On February 7, 2001, Employee #1 traveled out to a location on the top flange of a structrual steel building approximately 90 ft at the sixth floor level without being tied off or sliding along the beam with feet on bottom flange. Once reaching the location, Employee #1 did not tie off and proceeded to bring up a rope which had a hammer attached. As he stood straight up, he lost his balance and fell possibly due to a gust of wind. The witness to the accident was on the fifth floor approximately 15 to 20 ft away. Employee #1 died.
