Abstract: Employee #1 climbed onto an unguarded single tier scaffold in order to place a screw into a wooden plate within the diameter of an overhead light dome in preparation for installing a metal light frame. The scaffold was erected adjacent to and above the second floor balcony railing. The scaffold measured 5-foot-4-inches in height, 7-foot in length and 5-foot-1.25-inches in width. Employee #1 either tripped or walked off the scaffold end, falling 19 feet to the floor. Employee #1 received head and arm injuries. A call was made to Emergency Services and a Bonita Springs fire engine and paramedic unit responded. Employee #1 was treated at the scene and then transported by ambulance to Lee Memorial Hospital in Ft. Myers, Florida. According to Medical Examiner's autopsy report, Employee #1 died from cranial cerebral trauma.
