Abstract: At approximately 9:00 a.m. on the morning of August 22, 2011, an employee of, Herback General Engineering, was unbanding a stack of 8 in. DR-18 CIS PVC 1120 235 water pipe 20 feet 6 inches long. The stack contained ten individual pipes, weighing approximately 160 lbs each. When the employee went to cut the banding, he was standing on a slightly sloped area on the north side of the pipe. The employee was using a 2 by 6 stick of lumber to support the pipe from shifting his direction. When the banding was cut away, the pipe shifted the opposite direction. When he went to try and stop the shift, he accidently kicked the 2 by 6 stick of lumber, supporting the shifting of the pipe his direction. When the 2 by 6 stick of lumber was kicked away, three of the 20 feet 6 inches long pipes rolled into the employee taking him down a small ravine approximately 24 feet. The employee got tangled up with the pipe and injured his right arm, causing a fracture.
