Abstract: On July 10, 2008, Employee #1 was using a 5-in. hand held grinder (Model and Serial number were unknown), to cut notches in an angled aluminum, at a work site. Employee #1 was holding the aluminum stock with his left hand and the grinder in his right hand. The cutting disc failed, causing the grinder to move to the left, across Employee #1's left hand. Employee #1 sustained lacerations on his left hand, which required surgery. He was hospitalized. The grinder did not have a guard or a side handle.
