Abstract: On December 14, 1998, Employee #1 was making width adjustments to a splitter saw machine with the saw blades under power and the side guard for the saw open. The hinged portion of the side guard facilitated frequent changing of the saw blades. The hinged portion of the side guard also had an opening for clearance of the square arbor end nut. The employee left the hinged portion of the side guard open because the arbor nut was hitting the side guard, resulting in sparks when narrow cuts were made. At the time of the accident, Employee #1 was making width adjustments for a narrow cut. He failed to shut the machine off and reached across the machine to access a dial adjustment on its back side and slipped. His left hand went into the saw blade, amputating the employee's thumb, two fingers, and the tip of the ring finger. The employee was hospitalized.
