Abstract: At approximately 6:45 a.m. on November 19, 2003, Employee #1, a helper/roof loader was working for a company that sold and delivered roofing supplies. Employee #1 and a coworker were unloading asphalt shingles from a conveyor belt on to a roof. Employee #1 was on the second story of a home near the ridgeline, at a height of approximately 26 ft when he slipped and fell approximately 20 ft to the ground. Employee #1 had one year experience and stated that he did not wear any protection because he was not provided with any. Employee #1 also stated that there were not guard rails, safety ropes, or rope grabs to keep him from falling. Employee #1 was hospitalized and treated for a fractured right talus and a compression fracture of the lumbar two vertebras.
