Abstract: At approximately 5:30 p.m. on January 27, 2005, Employee #1 was assisting other coworkers on various jobs within the plant, such as sanding, cutting and taking wood to different departments within the work site. Employee #1 was using a table saw, nicknamed as the "45-degree angle saw" by the employer and located in the Assembly Department. Model and serial numbers of the table saw were not available. The table saw was used to cut straight and angle cuts. Employee #1 used the table saw to cut a 45-degree angle cut on a 2.5-in.-wide by 24-in.-long piece of wood. Employee #1 adjusted the saw blade to 45 degrees by using the two adjustment wheels located beneath the table saw. Employee #1 stated that the table saw did not have a spreader and anti-kickback device in place and that he lifted the hood guard to prepare the saw. The employer stated that the guards were "always taken off to cut the angle." When the saw blade was adjusted at the proper angle, Employee #1 brought down the hood guard and began to cut the wood. Employee #1 stated that the blade was not completely covered. According to Employee #1, the wood kicked and surprised him. Employee #1 does not remember how his hand slipped and became caught in the blade. Employee #1 suffered severe saw injuries on his left thumb, index finger, middle finger and ring finger with an amputation of the middle finger. Employee #1 was hospitalized.
