Abstract: On May 2, 2006, Employee #1 was changing out hardware on a utility pole, a job that required existing cross members and hardware to be removed. Employee #1 had loosened bolts on some of the hardware and was moving down the pole to a position 2 or 3 ft below. He had to un-hook his safety belt to move past a wooden cross member. While he was moving, he grabbed the cross member, which was loose. Employee #1 lost his balance and fell over 30 ft to the ground.
