Abstract: At 3:30 p.m. on April 20th, 2007, Employee #1 was working for a flooring installation subcontractor that sold and installed various flooring materials, including wood, laminated, ceramic tiles, and other flooring, located in Culver City, CA. The incident occurred at a customer's residence, when Employee #1 was ripping in half a piece of floor board 2.5 inches wide, approximately 3 feet long, and 0.75 inches thick. As he was ripping the piece of wood, the blade hit a knot, causing the piece of wood to jump back toward him. Because he was, at the same time, pushing it forward hard, his hand was cut by the saw's unguarded blade. He amputated the middle and ring fingers on his left hand. He was not hospitalized. Contributing causal factors were the unguarded blade, a lack of anti-kickback and splitter devices, and not using a push stick.
