Abstract: At approximately 12:45 p.m. on June 18, 2001, Employee #1, a journey-man decker, was shaking up decking and tacking it in place. He worked for Wincon Incorporated, a subcontractor company involved with decking of levels at buildings undergoing construction, as a temporary employee. The decking work Wincon Incorporated was performing was subcontracted to them by DNM Steel, the steel erection company on site. The mezzanine level is located thirteen feet six inches from the ground level. Wincon Incorporated was subcontracted to install the corrugated decking on this level. The galvanized, corrugated decking dimensions are three feet by twenty five feet and contained male and female edges for connecting the sheets with each other. The decking is initially placed at one end of the mezzanine level, stacked and ready for spreading. The act of spreading the decking in their appropriate positions on the mezzanine level is known as "Shaking it out" in the construction industry. The job duties of Employee #1 were to shake up the decking and tack them in place. Other employees are responsible for following Employee #1 to weld the decking in place. When there is no decking in place yet, the Wincon Incorporated employees either "cooned" the I-beams or walked on top of them. There was no fall protection provided for Wincon Incorporated employees as they performed their job at the accident site. Employee #1 was a journey-man decker and had been decking for 2.5 years prior to this incident. The Wincon Incorporated crew had been working all morning. Coworkers believe he slipped and fell through the opening between the wall and the decking but unfortunately could not recall any details from falling or hitting the ground. Employee #1 had musculoskeletal pains when he fell 13.5 feet from the mezzanine to the concrete floor while decking the mezzanine level. Employee #1 was hospitalized at Harbor UCLA General Hospital where he stayed for two days for treatment of musculoskeletal pains.
