Abstract: At 10:30 a.m. on July 16, 2003, Employee #1, a full-time group leader, employed by Jackson Charvel Manufacturing Inc., in Ontario, California, sustained a perforated bowel when the melamine coated particleboard he was cutting kicked-back towards the end of his cut, spun and struck Employee #1 on the abdomen. Employee #1 was using an unguarded table saw, identified as Unit Number 10 at the mill department, to cut the stock made of melamine coated particle board, approximately 10-feet by 3-feet by 1-inch that was used for a veneer press. The mill department was the area where employees cut down materials such as melamine coated particle boards to desired size for making guitars, and contains presses used for veneer applications. At the time of the accident, a coworker was assisting Employee #1 by holding the hanging end of the board while Employee #1 was cutting the board to the desired size, 3-feet by 2-feet, for veneer press application. Employee #1 was pushing the board towards the end of the cut, when he lost control of the board. The board kicked-back, spun and struck Employee #1 on the abdomen and threw Employee #1 back approximately 4 feet. Employee #1 sat for a little while at the lunch room until the Manager convinced him about an hour later to see the company physician, where he was treated and released. Employee #1 went to the Kaiser Foundation Hospital in Fontana the following day for a follow-up examination and the examining physician determined that Employee #1 had sustained a punctured small intestine which required hospitalization for 11 days.
