Abstract: At approximately 9:45 p.m. on November 6, 2001, Employee #1, a Cuber Machine Operator, employed by ORCO Block Co. Inc., Romoland, California, had shut down a Besser 80-cubic-foot cement mixing machine, applied lockout/tagout, opened the rear access door and started cleaning the machine with a compressed air hose before the mixer blades came to a complete stop, when the rotating mixer blades crushed the employee's arm between a blade and the mixer's frame. Employee #1 sustained a crushing fracture to his right forearm. Employee #1 was taken by a coworker to the Valley Plaza Doctors Hospital in Perris, California, where he was admitted and hospitalized for four days.
