Abstract: At approximately 8:00 a.m. on March 14, 2001, Employee #1, a packaging machine operator, was in the process of cleaning the deposited corn from the inner side of a "BOSTON GEAR, Series-100" pre-wash corn mixer machine. Employee #1 was cleaning the machine manually with her right hand when her hand slipped and came in contact with the moving screw augur causing amputation of her right index finger. The pre-wash corn mixer machine did not have a cover or enclosure with an interlocking device so that power cannot be applied to the agitator unless the cover or enclosure is in place on the mixer. Employee #1 was hospitalized.
