Abstract: At approximately 12:15 p.m. on September 21, 1998, an employee sustained a partial amputation of the left thumb. The employee was attempting to replace cable that had come off of a sheave. The employee had extended the 15 ft slide-out-boom, contained within the 60 ft boom of a 10-ton, BLH Austin Western, Model 410 crane. When the boom was extended, the cable lifted off of the top sheave, causing the cable to unload. There was no close-fitting guard or other suitable device to guide the rope back into the groove when it is unloaded.
