Abstract: At approximately 11:30 a.m. on October 6, 2004, Employee #1, working for the DRI Commercial roofing company in Foster City CA, heard the engine on a Cleasby model #845 tar kettle, unit #11851-11-845, with California license plate #SE 505408, sputtering. He checked the fuel level and, finding it sufficient, next checked the spark plug. After he cleaned the spark plug, the engine started operating normally. As he was walking off the top deck he slipped, so he used his right arm to brace himself. His right hand landed on the Lovejoy-type shaft coupling, with the sleeve of his shirt catching the rotating shaft. His right arm became entangled with the rotating shaft, and as a result, he suffered a compound fracture of his right arm. It is unclear if Employee #1 removed the guard on the machine, and he could not be reached to clarify whether the machine was guarded or he removed the guard at the time of the accident. There was no response even though several calls were made and a letter was sent. No citations were issued. The accident was reported by the Foster City Police Department on December 2, 2004, at 8:00 a.m.
