Abstract: Employee #1 was transporting an 800-lb liquid oxygen cylinder using a specialized hand cart. The cart has a hook that attaches to the top of the cylinders during transport. However, the cylinder that was delivered was larger and heavier than normal. Employee #1 placed the oversized cylinder on the cart and attached the hook. Due to the size and weight of the cylinder the hook could not hold the cylinder in place and snapped off causing the cylinder to fall of the cart. Employee #1 tried to jump out of the way and somehow got his right third and fourth finger caught between the cart and the cylinder. Employee #1 sustained crushing injuries to his right third and fourth fingers. Employee #1 was taken to the hospital where he was treated for his injuries. Doctors were required to shave off the bone in order to repair Employee #1's fingers. Employee #1 was not hospitalized.
