Abstract: At approximately 4:45 p.m. on July 22, 2002, an employee and a coworker, of Volt Services and under contract to Leiner Health Products, were cleaning a Kamata tumbler; model HB-1, serial number SG-5010. The Kamata tumbler was a machine that encapsulated medication into soft gelatin capsules. Leiner Health Products had 80 Kamata machines at the time, and this was the sixtieth machine that the employee and the coworker had cleaned that day. To clean debris that had fallen between the tumbler drum and the lower panel of the Kamata tumbler, the employee removed a chain guard cover to clean it with a rag. His cloth became caught on the exposed chain and sprocket, which pulled in his right hand, amputating the tips of his right index and middle fingers. The employee was hospitalized for treatment. The employee was trained by Leiner Health Products staff and had been assigned to clean the bottom of the tumbler by his supervisor, a senior mechanic. Leiner Health Products maintained supervision and control over the contract workers it hired from Volt Services.
