Abstract: On August 20, 2002, Employee #1, a sanitation worker, was using chemical foam to clean the whole tomato peeler. As she was working, the chemical foam cleaning system, which mixed the chemical and water automatically and sprayed it from a hose, began to lose pressure. The original nozzle did not contain a valve, but the chemical supplier was installed to increase the system's efficiency. As pressure was regained, the fitting that was wedged into the hose came lose. This resulted in a sudden increase in pressure, causing Employee #1's hand to fly up. The spray nozzle struck her safety glasses and knocked them off, and the nozzle hit her eye. She was severely injured and was hospitalized. The accident was the result of the valve functioning erratically.
