Abstract: This employer operates a trucking company. They were in the process of hauling logs to a sawmill. Employee #1 works for the company as a truck driver. Employee #1 was helping hook his trailer up to the fork of a front end loader. The fork has a hook on it that catches on to the loop on the trailer. Employee #1 was standing on the side of his trailer away from the loader. The fork of the loader was resting on the weigh pad on the trailer. This kept the fork from lowering as intended. Neither the loader operator nor Employee #1 noticed that the fork was resting on the weigh pad. The fork suddenly slipped off the weigh pad and crushed Employee #1's little finger on his right hand. Employee #1's finger was amputated by this action and he was hospitalized.
