Abstract: On September 28, 1998, Employee #1, a laborer for a highway paving company, was walking backward in front of the steering tires of a Blaw Knox road widener, holding a shovel in the hopper of the machine. She either tripped and fell in front of the vehicle or her right foot became caught under the tires, causing her to fall. The road widener ran over the entire right side of Employee #1's body. She sustained massive crushing injuries, from which she died. Employee #1 had no training in the operation or hazards of this machine. The company had a health and safety program, but it was not enforced.
