Abstract: On November 28, 2001, Employee #1 of the Red Seal Feeds, LLC. was working around a "smooth" horizontal shaft, which was approximately 2 in. in diameter. Employee #1 helped clear a jam on a 6-in. wide flat canvas power distribution belt. The horizontal shaft was smooth and did not have key slots or other protrusions coming out of the shaft. The shaft had caked on grain dust, approximately 0.0312 in. deep in an irregular pattern running along the shaft. Employee #1's coat got caught on the revolving conveyor belt shaft as it was rotating and pulled Employee #1 into it, causing the coat to tighten up on the shaft and to crush the chest of Employee #1. Employee #1 was killed. The coat was not buttoned up on Employee #1, but hanging loose from his shoulders.
