Abstract: On April 10, 2008, Employees #1, the owner of the company, and #2 were testing a power panel for electrical power to troubleshoot a garage door that would not close due to a lack of electrical power. The Employees went behind the electrical panel where to 25 KV conductors entered the panel and removed several covers exposing the energized conductors and lugs. Employee #1 began testing the circuit with a 110 and 220 volt test meter. Employee #1 touched one of the lugs and a large flash and fire occurred which caught both employees clothes on fire. As they ran from behind the panel they extinguished one another then were assisted by other coworkers. Both employees were air lifted to the burn unit at West Penn hospital, and both employees were admitted with serious burns. Employee #1 died on April 30, 2008 due to complications from the serious burns.
