Abstract: Employee #1 was replacing a broken section of 12 in. drain pipe in an excavation whose depth varied from 5 1/2 ft to 7 ft. He was preparing to put gravel in the base when a portion of the wall caved in. Employee #1 was buried to just below his knees, a depth of between 15 and 18 in. He was immediately removed from the excavation and treated by medical personnel. Although he had no apparent serious injuries, and he was conscious and alert the entire time, Employee #1 complained of pain in his chest and rib cage. He was transported to the hospital, where he died four days later from compression of the chest. It is presumed that a portion of the collapsing wall struck Employee #1 in the area around his rib cage. A protective system had not been installed in the excavation to guard employees in the event of a cave-in.
