Abstract: An employee was rigging pipes so that they could be offloaded from a tractor-trailer. He was using a 51-millimeter carbon steel stanchion to wedge an 11.6-meter-long, 406-millimeter-diameter pipe weighing about 3440 kilograms away from another pipe to make room for the rigging around the pipe. As he was doing this, the pipe began to slide and rolled off the trailer. The employee tried to jump free, but the pipe followed the employee off the truck and landed on his back, neck, and head. The employee died of his injuries.
