Abstract: Employee #1 was standing on a 40 in. by 48 in. wooden pallet that was elevated between 11 and 12 ft by a Clark forklift. He was attempting to store extra merchandise on the top level of a shelving unit when he lost his footing and fell onto the concrete floor. Employee #1 was killed. Subsequent investigation revealed that apparently a wooden slat had broken on one of the pallets, causing him to lose his balance.
