Abstract: On November 14, 2001, Employee #1 and another iron coworker were positioning structural support beams on the roof of a building. As they were securing the support beams into the structural beams of the building, Employee #1 lost his balance. He fell approximately 20 ft. Employee #1 was not wearing any fall protection equipment. He struck the ground caused traumatic head injuries, which were fatal.
