Abstract: Employee #1 and a coworker were performing their duties while four, approximately 500 lb columns were being unbolted from a paint pallet and lifted onto a truck for shipping. Three of the four columns had been completely removed from the pallet when the weight of the last column caused the pallet to tip over. The column, which was still secured to the pallet, struck Employee #1 and his coworker, pinning them underneath it. Employee #1 sustained a fractured hip and internal injuries, while the coworker sustained minor cuts and bruises. The columns were bolted by the flange onto the pallet so as to project upwards, similar to an upside-down table. The columns were secured this way to make them easier to paint.
